Organization Background:
Established in 2002, Precious Blood Ministry of Reconciliation (PBMR) is a faith-based, restorative justice organization in the Back of the Yards/Englewood community.  PBMR is rooted in the spirituality of the Precious Blood, we restore human dignity through hospitality, hope, and healing.  Through a restorative justice philosophy, we seek to build a stronger community.

We work as agents of reconciliation to:
  • Build relationships among youth and families impacted by violence and/or conflict
  • Create safe spaces where people can experience radical hospitality, hope, and healing
  • Promote a restorative approach to conflict and build a sense of community.

Position Summary:
The Case Manager works with a caseload of participants to assist them towards becoming productive, active members of the community. Case management for our participants includes but is not limited to: support related to employment and education; life skills training; crisis intervention; relationship training; advocacy; support with involvement in the justice system; and assessment of and referrals to needed resources. Services will be provided in individual, family, and group contexts. Case Managers are to have a maximum caseload of fifteen (15) participants.

Essential Functions:
The essential functions of the position include, but are not limited to, the following:
  • Identifies and develops relationships with in-risk individuals within target community
  • Interviews participants to assess and determine need for additional services
  • Coordinates between internal and external services provided to participants; identifies and arranges resources
  • Maintains relationships by verifying participants' attendance; observing and evaluating activities and responses; advocating for needed services; identifying and connecting to additional resources; intervening in crises; and providing personal support
  • Supports and teaches, as necessary, various job readiness workshops to individuals and groups, including computer skills, communication, critical thinking, self-awareness, conflict mediation, financial literacy, job search and application techniques, resume and cover letter development, interview skills, etc.
  • Inputs data into participants' records consisting of case notes, assessments, and events daily in online data systems
  • Communicates participants' progress by engaging in weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to team and family, and identifying program influences
 
  • Prepares participants' exit strategy by reviewing goals and individualized plans, coordinating discharge and post-discharge requirements, orienting, and training family members, and providing resources
  • Develops partnerships with resources that will strengthen support for participants and holds partners accountable as needed
  • Case Managers are mandated reporters and are expected to work at high level of team engagement
Qualifications:
  • Bachelor’s degree in Human Services field (i.e. Social Work, Sociology, etc.) or equivalent experience
  • At least 3 years of experience supporting and working with in-risk youth and street-involved youth and developing individualized plans for in-risk youth; awareness of trauma reactions.
  • At least 3 years of experience working with people from various ethnic, economic, and educational backgrounds both as colleagues and as clientele.
  • Experience documenting case notes.
  • Experience developing and retaining relationships with other service providers.
  • High level of interpersonal and conflict resolution skills.
  • Ability to communicate effectively both verbally and in written form.
  • Crisis intervention training or knowledge.
  • Knowledge of Chicago and communities with histories of violence, experience in Back of the Yards and Englewood neighborhoods preferred.
  • Proficient in Microsoft Office Suite and comfortable using computer programs to track participant engagement.
  • No pending criminal cases
  • PBMR is a drug-free and safe workplace, passing a drug screening and background check is required prior to being hired
  • Valid driver’s license and safe driving record; preferred personal vehicle with current insurance
  • Participate in and successfully complete the Virtus Training (i.e., Archdiocese of Chicago Safe Environment Training).

Schedule/Travel: At times, this position requires working evenings, weekends, and occasional travel.

Salary: $45,000 - $50,000
